A 58-year-old woman who underwent partial cystectomy for a high-grade invasive urothelial carcinoma of the left lateral bladder wall 11 months ago was referred of FDG PET/CT scan to evaluate a possible vaginal malignancy detected on a follow-up pelvic MRI 5 days ago. FDG PET/CT showed intense activity of the vaginal mass indicating malignancy and no other lesion suspicious for malignancy. A diagnosis of metastatic high-grade urothelial carcinoma of the vagina was made after removal of the mass.
