Women with ovarian metastasis from endometrial cancer are more likely to have >50% myometrial invasion, high-grade non-endometrioid tumors, and lymphovascular space invasion. Low-grade endometrioid endometrial cancers with <50% myometrial invasion and negative lymphovascular space invasion have a very low rate (0.5%) of ovarian metastasis. We describe FDG PET/CT findings in a case of low-grade endometrioid endometrial cancer with <50% myometrial invasion, negative lymphovascular space invasion, negative lymph node metastasis, and synchronous isolated unilateral small ovarian metastasis. Both the primary and metastatic tumors showed intense FDG activity.
