Endometrioma and Mature Cystic Teratoma as Collision Tumors of the Ovary: A Case Series and Literature Review
The simultaneous presence of distinct histologic types of ovarian cysts in a single patient, known as “collision tumors,” presents a clinical challenge. In this study, we report 19 new cases of ovarian collision tumors involving both endometriomas and mature cystic teratomas (MCTs), and we review their characteristics alongside 30 previously reported cases. Among the total of 49 patients, the mean age was 32.28±8.07 yr, with 66.67% being nulliparous. The most common clinical symptom was abdominal pain (55.10%), while 12.24% were diagnosed incidentally. Primary infertility was identified in 14.28% of patients. Of the 44 cases with available surgical data, 32 underwent cystectomy and 12 underwent oophorectomy. In the 19 newly reported cases, 10 patients desired fertility preservation, yet only one achieved spontaneous pregnancy after surgery. Although both endometriomas and MCTs are benign ovarian lesions, their coexistence is rare. Tissue diagnosis is essential to exclude malignancy and guide follow-up, but preserving ovarian reserve is equally critical. Recognizing the presence of collision tumors can help physicians utilize radiologic methods more effectively and adopt a more proactive approach, safeguarding the future fertility and quality of life of these patients.
Diagnostic accuracy of tru-cut biopsy and acid cytology from patients operated with suspicious for ovarian cancer
SUMMARY OBJECTIVE: In women who are believed to have ovarian cancer but have a poor performance status or have advanced disease thought to be beyond the scope of primary cytoreductive surgery, neoadjuvant chemotherapy can be administered with acid cytology and/or tru-cut biopsy referral. The aim of this study was to determine the accuracy, adequacy, safety, and reliability of these minimally invasive interventional procedures. METHODS: This is a retrospective analysis of 63 patients with a suspicion of ovarian cancer who reported to Bezmialem University Hospital between 2014 and 2021, underwent ultrasound-guided acid cytology and tru-cut biopsy, and also had postoperative final pathology results. RESULTS: On comparing acid cytology and tru-cut biopsy at the same time with the postoperative final pathology results, it was seen that the positive predictive value was 100% in all groups. It was revealed that the sensitivity of acid cytology was 64%, the specificity was 100%, the negative predictive value was 12%, and the accuracy of the test was 65%. The sensitivity of the tru-cut biopsy was 91%, the specificity was 100%, the negative predictive value was 42%, and the accuracy of the test was 92%. In the case of both procedures, the sensitivity was calculated as 93% and the accuracy of the test was calculated as 93%. There were no false-positive cytology and biopsy results. CONCLUSION: Due to its high reliability and accuracy, the combined application of these minimally invasive methods has the potential to routinely replace more invasive methods for adequate tumor sampling, such as diagnostic laparoscopy or exploratory laparotomy.
Knowledge levels and community guidance of doctors working in family health centers on HPV screening and HPV vaccination
AbstractAimOur aim is to evaluate the level of knowledge, compliance with the screening program, and tendency to inform patients of the doctors working in FHCs where HPV testing is performed within the scope of the cervical cancer screening program in our country.MethodsThis cross‐sectional study was performed between June and September 2022 with 113 family physicians working in different FHCs in different provinces in Turkey. Questionnaires prepared by the researchers were delivered to family physicians via online platforms.ResultsWhen the different age groups were evaluated, in two of the 24 knowledge‐level questions (How many types of HPV are there? Can HPV infect men?) the rate of correct answers for participants under the age of 35 years was statistically significantly higher (p = 0.007; p = 0.032). With regard to professional experience, the group with fewer than 10 years of experience gave a statistically significant correct answer to two questions (How many types of HPV are there? Can HPV infect men?; p = 0.008; p = 0.037). It was observed that 107 (94.7%) of the 113 family physicians who participated in the survey recommended that their patients use condoms during intercourse, 110 (97.3%) recommended cervical cancer screening tests to patients who applied for another reason, 105 (92.9%) recommended the HPV vaccine to patients and their relatives and 60 (53.1%) recommended the HPV vaccine to patients who applied for another reason.ConclusionsThe success of HPV vaccination programs is directly related to the beliefs of health personnel and their recommendations to the general population.
